Thrive Youth Services LLC specializes in the design and implementation of intensive youth development services that aim to rehabilitate and guide youth ages 12 to 18 (up to 21 with extended care plan). We work closely with various medical professionals, psychiatrists, psychologists, independent living skills workers, and PCA service providers in running special mentoring programs and youth employment schemes that can help them better manage their conditions, get back on track, and improve the quality of their lives. We also provide one-on-one staff servicing for youth with higher behavioral needs and individualized home supports.
A program coordinator is responsible for a variety of duties related to the overall care of our clients as well as supporting the program manager in implementing programming for individuals. This can include providing transportation, encouraging person centered activities, ensuring their site is in compliance with 245D and managing the staff at that site. Duties and responsibilities will be detailed below.
*As a program coordinator, this role requires mandated reporting to the state of Minnesota regarding all suspected maltreatment of a minor or vulnerable individuals
DUTIES AND RESPONSIBILITIES:
Training
- Complete New hire Orientation
- Complete and maintain all required State mandated 245D trainings
- Complete and maintain required onsite and client specific trainings
- Attend all meetings and training sessions as required by 245D trainings and TYS
- Stay in compliance with required annual trainings
Communication
- Regular communication (E-mail,phone calls,in-person meetings) with Program Manager and additional management team staff. Communication with Client teams may be necessary at the request of your Program Manager.
- Communicates any changes to client safety, health, behavior, and or needs to the necessary parties
- Effectively communicates important information within the agency in a timely manner
- Effectively communicates important information, client, and program updates with DSP staff. Will host staff meetings at the request of the Program Manager.
- Remains respectful and professional within all agency related communications
Documentation
- Program Coordinators are required to check the MAR (Medication Administration Record) multiple times a day for errors and corrections.
- Assist with and/or completes staff scheduling
- When involved in an incident, the PC will complete an incident report. If not involved, the PC will assist involved staff in completing the report in a timely manner. PCs will also review and edit incident reports prior to submitting them to Program Managers.
- PCs will keep a detailed record of incidents and concerns related to staffing and/or clients at their site
- PCs will keep record of clients care teams, contact lists, and client need-to-knows
- Enters daily note in Therap describing interactions with staff and clients, concerns, observations and any major incidents from the day
Staff management
- Completes and follows up on all staff disciplinary action in a respectful and timely manner.
- Provides staff with clear and concise program schedules, expectations for site, and processes to be followed
- Monitor and document staff performance
- Provides ongoing staff training as needed to constantly improve site structure and moral
- Consistently conducts themselves in a professional manner at all times
- Address staff conflict in an unbiased and resolution focused way
- Manage DSP ability to assist individuals with ADLs and provide support when needed
- Monitor staff notes in Adaptive for inconsistencies, concerns, and completion of programming tasks
Safety
- Ensures that all staff are aware of and know how to execute all agency protocols
- Executes emergency response drills
- Immediately informs management staff of any incident regarding staff and client health and safety
- Maintains a site that follows all safety regulations and codes.
Programming
- Monitors and implements a structured system pertaining to medication administration and documentation. Actively communicates and collaborates with the agency Nurse regarding all medications and health needs/concerns.
- Oversees the implementation of client goals and outcomes
- Works closely with the activity and event coordinator to create and implement activity schedules
- Completes and oversees daily schedules, ensuring that they are in coordination with the needs of each individual client
- Hosts monthly staff meetings to announce updates and provide ongoing training
- Ensures that all clients’ mental health, medical/dental, and educational needs are being met
- Collaborates with clients’ care teams to ensure appropriate care of the client

- Prepare paperwork, schedule, and facilitate 45-Day, semi-annual, and annual meetings

Requirements
- High School Diploma
- Valid driver’s license and clear driving record.
- Ability to drive company vehicles.
- One to two years’ experience working with teens and/or people with developmental disabilities.
- Ability to lift/push/pull 50lbs.
- Ability to work on holidays and during inclement weather.
